چکیده

Radiosurgery is a common approach for patients with brain tumors. After stereotactic radiosurgery (SRS), the differentiation between tumor progression and radiation effects (RE) can be difficult using current clinical criteria supplemented by computed tomography (CT) or magnetic resonance (MR) imaging (MRI). This is one of the greatest challenges in radiosurgical practice and indeed in neurooncology. Metabolic imaging with single-photon emission computerized tomography (SPECT) or positron emission tomography (PET) also fails to provide reliable predictive information. We studied whether selected clinical and MRI criteria can reliably differentiate tumor progression from REs in patients who required resection of their brain mass. Differentiation between tumor progression and REs is a fundamental challenge that directly affects patient care. Desquesada et al recently reported that their novel radiographic feature, called the ‘‘lesion quotient,’’ which was the ratio of the nodule volume as seen on T2-weighted MR imaging to the total-enhancing area on T1-weighted MR imaging, was significantly associated with tumor recurrence or REs. We focused on the correlation of any nodular lesion margin (not the measured lesion volume) on T2-weighted MR imaging compared to T1-weighted enhanced imaging.

Stereotactic radiosurgery as a primary treatment modality for pineal parenchymal tumors

Background: Pineal parenchymal tumor (PPT) is extremely rare and histologically heterogeneous, and the optimal treatment strategy is controversial. This study examined clinical outcomes of patients with PPT treated with cyberknife radiosurgery (CKRS) as a primary treatment modality after tumor biopsy. Materials and Methods: Between 2009 and 2018, there were six patients diagnosed with PPT who w...

Whole-brain radiation therapy for brain metastases: detrimental or beneficial?

Stereotactic radiosurgery is frequently used, either alone or together with whole-brain radiation therapy to treat brain metastases from solid tumors. Certain experts and radiation oncology groups have proposed replacing whole-brain radiation therapy with stereotactic radiosurgery alone for the management of brain metastases. Although randomized trials have favored adding whole-brain radiation ...

Fractionated stereotactic radiosurgery with concurrent temozolomide chemotherapy for locally recurrent glioblastoma multiforme: a prospective cohort study

Local recurrence represents a significant challenge in the management of patients with glioblastoma multiforme. Salvage treatment options are limited by lack of clinical efficacy. Recent studies have demonstrated a significant response rate and acceptable toxicity with the use of fractionated stereotactic radiosurgery in this patient population. Our primary objective was to determine the effica...

BACKGROUND: Nonfunctional pituitary adenomas (NFPAs) represent approximately 50% of all pituitary tumors. OBJECTIVE: To evaluate the long-term outcomes of stereotactic radiosurgery for NFPAs. METHODS: We evaluated the management outcomes of Gamma Knife radiosurgery in 125 patients with NFPAs over an interval of 22 years. The median patient age was 54 years (range, 16-88 years). One hundred ten ...

Novalis stereotactic radiotherapy improved metastatic pituitary tumor.

An 86-year-old woman presented with anterior and posterior pituitary deficiencies 5 months after chemoradiation for small-cell lung carcinoma. She had achieved remission, but gadolinium-enhanced T1-weighted magnetic resonance imaging revealed a pituitary mass with suprasellar extension, which was unrecognized 3 months previously, indicating metastasis (Picture 1A) (1). Because she did not want ...
